Insurance Plans vs. Out of Pocket
Payment
Dr. Daniels accepts most insurance plans that allow the patient
to choose "any willing provider". It is important
to note that out of pocket patients enjoy complete discretion
and confidentiality. Most insurance carriers require the release
of progress notes and or treatment plans in order to approve
the patient for benefits. This information may be used for
a variety of purposes, including but not limited to litigation,
if subpoenaed.

Payment
Payment is generally due at the time services are rendered,
unless other arrangements have been made. If a patient decides
to pay out of pocket, a sliding scale may be negotiated.

Confidential Services
All details of your treatment are protected by "psychotherapist
- patient" privilege to the full extent covered by law
and professional ethics. Dr. Daniels limits her participation
in "managed care" contracts which require the release
of progress notes or treatment plans to any third party.

All services provided by Dr. Daniels are in full accordance
with the Patient's Bill of Rights.
